Client Bill of Rights
As a client of Counseling Consultants you have the right and responsibility to: Be treated with dignity and respect at all times. Ask for and receive information about a social worker's qualifications, including license, education, training, experience, membership in professional groups, special areas of practice and limits on practice.
Get accurate information and helpful resources.
Keep appointments and have them kept with you.
Have information, before entering services about fees and methods of payment.
Refuse to answer any question or give any information you choose not to answer or give. (However, this may affect your ability to adopt depending on the adoption resource used.) Know if your social worker will discuss your case with others, i.e. supervisors, consultants, or students.
Comply with requests for documents and information. All document and information requests are necessary based on state, federal or international regualtions. Your worker will not request any document or statement unless otherwise needed by these authorities. Please understand that these regulations or the interpretation of these regulations change from time to time.

Fees and Payment
Payment is due when invoiced or decided upon by you and your social worker. We reserve the right to recoup fees that have not been paid through the use of an attorney or collection agency.
Other services such as court related services (testimony, depositions, attendance at court hearings, etc.) are billed for the time involved. You should talk with your counselor if you need these services. Complaints/Issues All of the professional staff abide by all of the rules of their professions and state licenses. If you are not satisfied with the services you are receiving from Counseling Consultants or if you disagree with our plans and decisions, you have the right to appeal or make a complaint. You must first talk to your Counseling Consultants therapist/caseworker about the problem. If this is not possible or if you and your therapist/caseworker cannot work out the problem, you have the right to ask the therapist’s/caseworker’s supervisor of Counseling Consultants to help solve the problem. Please right down what is wrong so the supervisor will know how to help settle the problem. The agency may request a meeting with you (phone or face-to-face) or seek more information. If you are still not satisfied you have the right to ask the Executive Director of Counseling Consultants to help solve the problem. After additional fact finding, this individual will provide a concluding decision to you within 15 days. The response will be given to you in writing and a copy will be included in your case record. Counseling Consultants does not discriminate against clients because of any of these factors; race, religion, age, sex, marital/family status, or sexual orientation. This is our policy. |
